Transition Planning

• Transition Plan document — written specifically for your child
• One-page Setting Summary — for daily reference at the setting
• Attendance at one transition meeting
• 4 weeks WhatsApp support across the transition period
• One follow-up review call
Delivery: Remote with attendance at one in-person meeting (where local)
Engagement window: 4 weeks
Complex Transition Support

For transitions involving complex needs — children with medical needs, social care involvement, multiple professionals already engaged, or EHCP-specific transition planning — where co-ordinating the full picture matters as much as the planning itself.
What’s included:
• Discovery call and full intake
• Transition Plan document — written specifically for your child, integrated with input from involved professionals
• One-page Setting Summary — calibrated for the complexity in play
• Multi-agency liaison with involved professionals (with your consent)
• Attendance at the transition meetings needed across the engagement
• 6 weeks WhatsApp support across the transition period
• Two follow-up review calls
Delivery: Remote with attendance at in-person meetings (where local)
Engagement window: 6 weeks
In-Setting Support
£985
For families navigating a current setting where things aren’t working as they should — and you need help thinking it through and moving forward.
What’s included:
• Discovery call and full intake
• Confidential Family Brief — my honest assessment, for you alone
• Collaborative Planning Note — a partnership document for the meeting(s) with your setting
• Attendance at up to three meetings with the setting, scheduled as needed across the engagement window
• 6 weeks WhatsApp support
• One follow-up review call
Delivery: Remote with attendance at in-person meetings (where local)
Engagement window: 6 weeks
EHCP Parental Request
£395
For families ready to formally request an Education, Health and Care needs assessment from their local authority — and who want professional support drafting that request.
What’s included:
• Discovery call and full intake
• Drafted parental request letter, grounded in current statutory framework
• Preparation guidance and evidence checklist
• Review of the letter with you before submission
• 2 weeks of follow-up WhatsApp support after submission
